Paul,
Ik zie nergens in deze configuratie dat authenticatie is geconfigureerd. Dit zul je eerst moeten instellen, voordat het gaat werken. Zie http://www.postfix.org/SASL_README.html. Het meeste in dat document gaat over het controleren van authenticatie (smtpd), maar jij bent geïnteresseerd in authenticatie als client (smtp). Zie de sectie "Enabling SASL authentication in the Postfix SMTP/LMTP client".

Dus zoiets als:
smtp_sasl_auth_enable = yes
smtp_sasl_password_maps = hash:/etc/postfix/sasl_passwd

/etc/postfix/sasl_passwd:
[mail.xs4all.nl]              username:password
Zou best wel eens kunnen werken...
